CUPE Local 813: Employee/Family Assistance Program (EFAP)

We suggest you first contact the EAP program that the hospital provides You just need to ask

CUPE Local 813’s E.F.A.P. is there that assist members and there families in a time of need.

This program can put you in touch with the right people to help you on the road to recovery from drug and alcohol dependency, gambling addiction, family and personal matters as well as other events that may be effecting your life and work.

The program offers 5 sessions for members and their families per year. Although this is just be a start, the assistance that the local provides can help.

The only thing that hurts is not getting the assistance one may need. The assistance that the local provides is not a cure all but it is a step in the right direction and sometimes a little direction is all one needs.

There are referral agents that can assist you in contacting the agencies and all information given to the referral agents is held in the most confidence. If you are seeking help, contact one of our referral agents for details and to see if you qualify for this program.

Guidelines of the Employee/Family Assistance Program of Local 813

  • Counselling: Maximum Rate $80.00 per one (1) hour session, five (5) sessions in total, per member including family, per calendar year. Any amount above the maximum rate is the sole responsibility of the member
  • Coverage: E.F.A.P. is available to the members of CUPE Local 813 and their family members, (as defined as spouse or ward who lives within a members household and who is dependent on that household for support). Any member or family member who wants to utilize this program must be referred by one of Local 813’s Referral Agent
  • Other Services: Psychological Service – Local 813 will cover up to a maximum of $80.00 per one (1) hour session, five (5) sessions in total, per member including family, per calendar year. Any amount for this service above the maximum rate is the sole responsibility of the member
  • No Show: No fee will be applied for a “NO Show” appointment. This will be the responsibility of the member. We ask that you contact the “Referral Agent” if this does occur.
  • All request must go through a Referral Agent.
